Staff ID Badge Lost or Repair Needed

If you are an APS staff member and have either lost your ID Badge or your ID Badge is no longer working, please complete this form to schedule an appointment with the Engagement Center to have this issue rectified.

The staff ID badge process will be scheduled on Mondays and Fridays from 10 a.m.-2 p.m. in the Engagement Center (main entrance) of the Sylvester Small Administration Building at 10 N. Main St. You will receive a follow-up email with a scheduled appointment time and APS safety protocols to follow when reporting for your appointment.

The staff ID badge process is done in the most protective manner possible. The staff member remains in a private area prior to the staff ID badge appointment.

Please bring an ID with you, and wear a mask/facial covering.
