Dear APS Family:

We are asking for your input on an important matter for our scholars. In filling out this brief survey about students and how they get to and from school each day, you assist our efforts to make the trip safer for them.

The survey helps the city and Akron Public Schools in our pursuit of a grant for the APS Safe Routes to School program.  

Data collection information:

To help, simply answer a few questions about how your student(s) got TO and FROM school for any two days between today and Friday, February 2, 2024.  

You may print the form (see the list of forms below), or use the following Google form via the QR code:

If using the printed (manual) form, you can fill out two days on that same sheet. With the Google form, you will need to complete the form and submit it twice.
