
Akron Children's Hospital is providing School-Based Health Centers that offer private and confidential medical care at Akron public schools. With a parent/guardian's permission, APS students can receive:
- annual well visits
- vaccines
- sports physicals
- help managing asthma and allergies
- treatment for minor illnesses such as
- cold and flu
- fever
- sore throat
- ear infections
- pink eye
- rashes and poison ivy
- respiratory infections
These services do not replace a student's doctor but are an exciting, helpful resource for our families. Please complete this consent form and return it to your student's school to ensure they can participate in this program.
